Patient's demographics

Patient's full name (in capital letters( Age or date of birth, Sex and date is mandatory for all the test. Incomplete information will lead to delay in processing.

Reference

Mentioning of the name and contact number of the referring clinican and referral/collection center will help us communicating regarding patients finding, whenever necessary.

Test of Method
  • Test request out of the scope of DOT will I not be accepted.
  • Mention the test/profile name.
  • A test service may be discontinued without prior intimation.
  • The methods of analysis may change without prior notice.
Schedule, Cut off time and Date of Report
  • It will be our endeavor to follow the test schedule enclosed and furnish the reports on the committed time. However, the delay may occur for rechecks or any other unavoidable circumstances.
  • On individual request or due to any other circumstances, changing of schedule will not be possible.
  • Test cut off time and schedule may change without notice.
  • Tests are not carried out on Sundays and Holidays.
  • Part reports will not be sent.
Sampling

A. Collection and Separation

  • Serum or plasma must be separated before dispatch.
  • Whole blood samples need to be sent in vacutainers only.
  • Do not use "Serum/Plasma vials" for whole blood.
  • Request for analysis of insufficient, haemolysed, lipaemic, icteric, autolysed or poor quality samples WILL NOT BE ACCEPTED.

B. Labeling

  • Ensure the labels used, stick properly to the container.
  • Strictly use ballpoint pen or permanent marker only to label to ensure permanent marking.
  • Furnish complete information on the label i.e., patient's name, sampling data, sample type and test.

C. Storage and Packing

  • Use leak proof containers for body fluids, urine etc.
  • While collecting 24 hours urine, container should be maintained at 2-8c.

Additions, Alteration and Cancellation

  • After the samples are handed over to the courier boys, the test cannot be changed, withheld or cancelled.
  • In case an additional test is required from the sample sent previously, a written prescription is required within 5 days.
  • Test meant for "walk in patient only" have been clearly indicated in DOT. Such test samples, if sent, will not processed.
  • Unlabeled & incorrectly labeled samples will not be processed.
  • If it is not possible to mobilize new sample requested, the test would not be cancelled.
Logistics
  • Take special care while sending samples like tissues, products of conception, body fluids, baby's samples etc.
Test Charges
  • The prices listed are the charge to the patient.
  • Referral center is expected to charge as per the price list and not lower.
  • The prices are subject to change without prior notice.
Billing
  • A monthly bill statement will be sent on 1st week of every month.
  • Any discrepancy on the statement has to be clarified within 10 days of the receipt of the bill.
  • Please send us your statement of accounts, once in six months so that our books of accounts can be reconciled.
Payments
  • Please pay on time.

1. The payment are expected within 15 clays ofreceipt of the bill.

2. All the payment are to be madeby Cheque of DD favoring 'Swayamsiddh DiagnosticCare Pvt. Ltd., Jaipur'.

3. We recommend cashfor samples which are infrequent.

4. All the cash payment are to made on the same day.

  • Though we shall make sure to give the receipt of the payments, you are requested to pursue with accounts department in case thereceipt is not obtained.
